Vital Statistics staff serves as the primary resource to physicians, funeral directors, and the public for accessing information related to live births, deaths, and fetal deaths.
Hospitals in Washington State have 10 days to register the birth with the State of Washington. During this time, birth certificates are unavailable.

Request a Birth Certificate
- Fee: $30 per certified birth certificate.
- In-office request for birth certificate pick-up:
- Birth Certificates are available at our Coupeville office only.
- Days and times available: 8 am to 3:45 pm. Monday-Friday.
- Current Birth Certificate Application (pdf) of Jan. 1, 2021. Please see the application for acceptable identification.
Request a Death Certificate
- Island County Public Health can provide certified copies of any death occurring in Washington state from March 1, 2014, to the present.
- Fee: $30 per certified death certificate. Issuing certified death certificates in person at Island County Public Health, 1 NE 6TH Street. Coupeville, WA.
- In-office request for death certificate pick-up:
- Death Certificates are available at our Coupeville office only.
- Days and times available: 8 AM to 3:45 PM Monday-Friday.
